• Come with any questions for us regarding your treatment and financial needs

• Approximately 60 minute appointment
• Comprehensive examination including necessary x-rays
• Thorough description of findings and opportunity to discuss treatment and financial considerations

Existing patients must call the office to schedule all appointments.


We are in-network providers for select insurance companies (Delta Dental Premier, Paramount), accept most out-of-network insurance plans, and will strive to maximize your benefits. We currently do NOT accept state-funded dental plans (Medicaid, Healthy Indiana Plan). Please do not schedule a New Patient Appointment if one of these is your insurance provider.

Please note that you will be asked to indicate whether your visit is for an emergency or a comprehensive exam after initial information is entered. Additionally, your first visit with us will NOT include a teeth cleaning. Cleanings can be scheduled as a separate appointment following the completion of a comprehensive exam.
